چکیده

Objective Fetal endoscopic tracheal occlusion (FETO) has been reported to be beneficial to patients with congenital diaphragmatic hernia (CDH). It needs highly invasive procedures for balloon removal (fetoscopic and/or ultrasound-guided), and carries a given risk for both mother and fetus. We have reported the potential usefulness of high-intensity focused ultrasound (HIFU) as a noninvasive extracorporeal technology for disrupting intratracheal inflated balloon placed at FETO. In a few cases, it also caused skin burns or damage to the trachea after HIFU irradiation. The purpose of this study was to improve the optimum HIFU system as a noninvasive fetal treatment technology for disrupting intratracheal inflated balloon placed at FETO.

Successful tracheal occlusion using ultrathin fetoscopic equipment combined with real-time three-dimensional ultrasound.

OBJECTIVE The most common complication of intrauterine tracheal balloon occlusion is the preterm premature rupture of membranes (PPROM) which increases the rate of neonatal morbidity and mortality. Ultrathin fetoscopy may be a method of reducing the risk of PPROM. MATERIALS AND METHODS The operation was performed at the 27th week of gestation after sedation and relaxation of a fetus with bila...

Pathophysiological mechanisms of high-intensity focused ultrasound-mediated vascular occlusion and relevance to non-invasive fetal surgery.

High-intensity focused ultrasound (HIFU) is a non-invasive technology, which can be used occlude blood vessels in the body. Both the theory underlying and practical process of blood vessel occlusion are still under development and relatively sparse in vivo experimental and therapeutic data exist. HIFU would however provide an alternative to surgery, particularly in circumstances where serious c...

Fetal surgery for severe congenital diaphragmatic hernia?

Some fetal abnormalities, such as abdominal wall defects, are amenable to postnatal surgical correction with good results1. In other conditions, however, the survival rate after postnatal surgery is relatively poor. In congenital diaphragmatic hernia (CDH), for example, this is because intrathoracic herniation of the abdominal viscera compromises the development of the fetal lungs, this being e...

Severe diaphragmatic hernia treated by fetal endoscopic tracheal occlusion.

OBJECTIVES To examine operative and perinatal aspects of fetal endoscopic tracheal occlusion (FETO) in congenital diaphragmatic hernia (CDH). METHODS This was a multicenter study of singleton pregnancies with CDH treated by FETO. The entry criteria for FETO were severe CDH on the basis of sonographic evidence of intrathoracic herniation of the liver and low lung area to head circumference rat...

Pulmonary Vein Isolation by High Intensity Focused Ultrasound

Pulmonary vein isolation (PVI) using radiofrequency current (RFC) ablation is a potentially curative treatment option for patients with atrial fibrillation (AF). The shortcomings of the RFC technology (technically challenging, long procedure times, complications) steadily kindle the interest in new energy sources and catheter designs. High intensity focused ultrasound (HIFU) has the ability to ...
